Governance-grade questions, asked on a cadence
A working instrument for putting governance-grade AI questions on your board's agenda by design. It covers five domains of oversight — accountability, failure detection, human oversight, regulatory exposure, and information and reporting — each organised by when to ask: standing questions for every relevant meeting, formal-review questions for the deeper annual look, and trigger questions held ready for defined events.
The governance task is to understand what AI is deciding, what it is influencing, and what happens when it is wrong, and to ask the questions that make management account for all three. A vague answer is itself an answer: it tells you the thing you asked about has not been built.
